Established in 1996, Transitional Lifestyle Community, LLC (TLC) is a State licensed service provider under the New Mexico Developmental Disabilities Waiver Program. TLC was created with the belief that services for people with developmental disabilities should and can be provided in a professional and ethical manner. Meeting the unique needs of our clients without the challenge of layers of management and bureaucracy standing in the way is one of TLC's Top Priorities!  

 

TLC is a small organization with a “family” feel, where everybody knows everyone else. We believe that by remaining small and providing a limited number of services; we can offer HIGH QUALITY SERVICES that are customized and personalized to our client’s needs.

Is to assist Individuals with developmental disabilities in reaching the maximum level of independence consistent with their needs, abilities, and dreams, in a safe, secure, and caring family environment.

 

We promise to provide to every Individual receiving services:

  • A safe, clean, comfortable, and well-maintained living environment.

  • Individual and/or group training and documentation designed to assist in the development of attitudes and skills which support independence and success.

  • Caring, dependable and skilled staff and families who provide companionship, support, and assistance which respects every person’s dignity and rights.  

  • Comprehensive documentation and ongoing assessment of needs and services in order to ensure an Individual's development, satisfaction, and success.

  • Comprehensive support, including 24 hour on-call!

Individuals and/or families in a home setting and involvement in family activities

40 Hours of Initial training; 10 hours of training every year thereafter

TRAINING

Transitional Lifestyles Community LLC provides all State mandated training and certificates for FREE to all of our employees and subcontractors. TLC creates training calendars to allow staff as much notice as possible so you can make arrangements to attend. 

Training is required by the State and classes are held at TLC’s administrative office or via Zoom by State certified instructors.

 

As required by the New Mexico Department of Health, TLC offers all training courses.

Transitional Lifestyle Community, LLC is always looking for responsible, compassionate, House Supervisors, Residential staff, and Family Living/Mi Via/CIHS Supported Living Coordinators.
To become one of our caregivers, you must:

  • Be at least 18 years old

  • Have a high school diploma or GED

  • Pass a criminal background screening

  • Meet all training requirements - Free training provided

  • Have a valid New Mexico driver's license
